NOAA Fisheries Service, in cooperation with Pacific Northwest fish management agencies, has developed a template for completing HGMPs. It should assist applicants in meeting the criteria included in the final 4(d) rules. HGMPs appropriately completed using this template should include information needed to meet requirements of other ESA review processes (such as Section 10 permits), and to fulfill other non-ESA hatchery evaluation processes.
The HGMP template is available for download in the following formats. This is the August 2002 version; it represents only minor editorial changes from the Dec. 31, 1999, version (see description below of U.S. Fish & Wildlife Service "Addendum A"). HGMPs prepared before August 2002 may be revised to include this addendum.

The U.S. Fish & Wildlife Service has prepared an addendum to the HGMP template. This addendum should be completed for each HGMP whose activities may impact ESA-listed aquatic and terrestrial species under Fish & Wildlife jurisdiction. The addendum template was designed to complement the HGMP and to facilitate processing when evaluation of Fish & Wildlife-jurisdiction species is required.
